|Findings: The Surveyor reviewed seven emergency cards and found the following: a) Child 1 was missing an emergency contact, physician's phone number and a date of enrollment. b) Children 2, 4, 5, and 7 were missing a date of enrollment. c) Child 3 was missing an emergency contact and the name of the individual to be contacted in case of injury or sudden illness of the child. d) Child 6 was missing a date of enrollment, physician's phone number, the back of the card was blank and a parent signature was missing. |Findings: The Provider was not maintaining the emergency cards for the current enrolled children in a place that provides a staff member ready access to the card in the event of an emergency at, or evacuation of, the child care group home. The Provider stated the cards in the book the Surveyor was reviewing were not all currently enrolled but was unable to locate other emergency cards.|
|Findings: The Surveyor found an immunization record was not available for Children 1, 2, 3, 4, 5, 6 or 7. Previously cited on 10.23.18|
|Findings: The Team Leader and Surveyor observed eight enrolled children present but the Provider only had a sign in and out sheet for four enrolled children present. Children 8, 9, 10 and 11 were present but a sign in and out record was not available. Previously cited on 10.23.18|
|Findings: The Team Leader found a bottle of Ibuprofen on the kitchen counter.|

|Findings: The weekly schedule posted stated screen time is only provided on Friday for movie time. Both of the televisions were on when the Department arrived and the downstairs television was on when the Department left. Previously cited on 10.23.18|

|Findings: The Team Leader and Surveyor found the fire extinguisher in the kitchen was not mounted. The provider hung it on the mount during the inspection. Previously cited on 10.23.18|
