Findings: Upon arrival to the Home, Surveyor #1 and the Team Leader rang the front door bell and knocked several times, but no one answered the door. Children could be heard in the backyard, so Surveyor #1 went to the side gate and stated who they were and Staff #1 opened the gate. Staff #1 had a total of 4 children with them ( 3 enrolled children plus the provider's child). Surveyor #1 informed Staff #1 that the Team Leader was waiting at the front door and needs to be let in the Home. So, Staff #1 took one enrolled child with her and left the other 2 enrolled children with Surveyor #1. When the Team Leader entered the home there were 3 enrolled children observed in the napping room with the door closed. One of the children was asleep in a crib, one young child in a Pack N Play awake and the third child was an infant awake in a crib. There was a 4th child observed asleep on the couch. All 4 children were unsupervised as Staff #1 had been outside with the other children. An enrolled child went to the mini fridge in the main activity area and got a cup out and asked Staff #1 for some water. The two cribs in the napping room were lacking sheets and one enrolled child was observed sleeping in one of the cribs and another child was in the other crib, but was taken out when the inspection began.
