|Findings: Based on Surveyor observation and interview with the Provider, it was found that there were 3 Tiki Torches on the East side of the outdoor backyard accessible to the enrolled children.|

The provider stated that the fire extinguishers for the group home were bought in April 2017. It was determined that the fire extinguishers had not been serviced.|

|Findings: Based on the Surveyors' observations and staff interview, the mat used for diaper changing was no longer sanitizable because there was peeling tape on the edges of the mat. The provider stated the mat was torn, so she used the tape to cover the torn areas.|
|Findings: Based on facility documentation and the Surveyor's observations, it was determined that 2 out of 4 enrolled children's attendance forms were incomplete and lacked the following required information: Child #1: A release signature and time for 11/10/16. Child #2: Complete parent release signatures throughout the month of November 2016.|
|Findings: Based on the Surveyor's observations, there was a large wooden pallet located on the ground in the back yard accessible to enrolled children.|
|Findings: Based on facility documentation, the Surveyor's observations, and staff interview, it was determined that a staff member who had current training in first aid and CPR was not present in the home at the time of the inspection. The provider's first aid and CPR cards had expired, and she was the only staff member on the premises. The provider stated she was not aware that her first aid and CPR cards had expired.|
|Findings: Based on facility documentation and the Surveyor's observations, it was determined that the Emergency, Information, and Immunization Record card for child #1 lacked documentation of the name and contact telephone number of the child's physician.|
|Findings: Based on facility documentation and the Surveyor's observations, it was determined that the attendance form for child #1 was incomplete and lacked the following required information: * An admission signature/time for 06/22/16; * An admission time for 06/17/16; and * A release time for 06/09/16.|
|Findings: Based on the Surveyor's observations, it was determined that there was a large particle board panel laying on the ground in the northeast corner of the backyard accessible to enrolled children. The wooden panel was unsealed, and the wood was splintering around the edges.|
