The discipline policy was not followed. A caregiver yelled at children when frustrated, and physically forced children to sit down by yanking at children's arms when their commands were not followed. According to a caregiver this occurred daily for approximately a week. Caregiver also indicated observing another co-worker pop the hands of a child for not listening.

Child was handled roughly. At least three to four children's arms were yanked by a former caregiver on an off for approximately for a week before staff reported their concerns to administration. The caregiver is no longer employed as of January 27, 2023.

A child was yelled at, shamed, humiliated, frightened, threatened or bullied. Children in the Polar Bear room were yelled at, shamed, humiliated and frightened by a former caregiver. Three staff interviewed provided examples and frequency of the incidents.

Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The annual inspection was due no later than 10/12/2022. Exit lights are in need of batteries that are on back order.

A staff person confirmed screen time being used with preschool children, but not documenting the screen time amounts on a cumulative log.
