Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 06/18/2025. A copy of the Notice of Inspection Rights was given to the Provider. Please submit the Plan of Correction in the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Empower Survey link and Emergency Disaster Contact Form was emailed to the Provider.
