The provider was alone with 10 children at the time. There was no monitor to alert the provider of the baby's sounds.|
|Findings: Based on surveyor's observation, it was found that there were 2 packages of breast milk with first name only in the refrigerator. There was a bottle with initials only also in the refrigerator.|
|Findings: After a review of 10 children's Emergency, Information and Immunization Record cards, it was determined the following were lacking: Child 1: 1 of the 2 required emergency contact names and phone numbers. Child 2: date of enrollment.|
|Findings: After a review of the children's attendance records, it was determined the following were lacking: Child 1: signed in and out with initials in the month of January 2020. Child 3: no time of arrival or signature on 1/22/2020; no departure signature on 1/22/2020. Child 4: signed in and out with initials in the month of January 2020.|
|Findings: Upon arriving at the small group home, the surveyor observed 5 enrolled infants present and only 3 cribs. The surveyor obsevred an infant sleeping in a swing.|
|Findings: Findings include: The Surveyor observed an enrolled infant spend more than 30 consecutive minutes of time while awake in confining pieces of equipment. Upon the Surveyors arrival, an infant was awake and confined in a bouncy chair in the bonus room; midway through the inspection the child was still awake and in the bouncy chair.|
|Findings: At the time of the inspection, 5 infants were present and there were no feeding instructions, as required.|
|Findings: After a review of 2 staff files, it was determined the following were lacking: Staff 1 (start date 3/15/2017) had no documentation of the Central Registry Direct Service Affidavit. Staff 2 (start date 2/26/2018) had no documentation of the Central Registry Direct Service Affidavit.|
|Findings: After a review of 2 staff files, it was determined the following were lacking the DCS Central Registry background check results, as required: Staff 1 (start date 3/15/2017) Staff 2 (start date 2/26/2018)|
|Findings: Upon arrival at the home, the provider was not present. Staff 1 stated the Provider is at a doctor appointment. There was no documentation of the provider's start time or when they left the home. There were no attendance records available for Staff 1 or Staff 2.|
|Findings: After a review of 2 staff files, it was determined the following were lacking: Staff 1 (start date 3/15/2017) had 0 hours of the required 12 hours of annual training for the 3/15/2018-3/15/2019 time frame. Staff 2 (start date 2/26/2018) had 5 hours of the required 12 hours of annual training for the 2/26/2018-2/26/2019 time frame.|
|Findings: After a review of 6 children's Emergency, Information, and Immunization Record cards, it was determined was lacking: Child 1: health care provider's phone number.|
