| Findings: At the time of investigation, the facility was within ratio and proper supervision used. However, the facility recently had a child leave the playground to use the restroom inside and was not located for an extended amount of time. The child was later found inside in a classroom unsupervised. Resolution: Supervision guidelines and requirements were discussed with provider. Moving forward, the facility was assist children inside to use the restroom and will not leave them unsupervised. | |||
