Findings: Based on the Surveyor's observations and staff interview, the licensee did not not submit a written request to change the facility's space utilization. During the inspection the owner stated that the 2/3-year-olds classroom is now being used as a School-age classroom, and removing the diaper changing table. The 3-5-year-olds classroom is now being used as a 2's and up classroom. The licensse moved the diapering changing table into that classroom, however it wasn't previously approved for diapering. This occurred with out notifying the Department at least 30 days prior to the change. | |||
