Correction Required: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Description: General Health and Safety/Individual Records

Noncompliance Area: During the renewal inspection, the owners husband,who is in change of maintenance, was interacting with children during naptime and he has no file or clearances on site.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children. An individual record is required for each facility person.

Description: Emergency plan/Emergency plan

Noncompliance Area: During the renewal inspection, certification representative reviewed the facility files and the emergency plan did not include specific information on how staff are to respond in the event of an evacuation,relocation or lockdown.

Correction Required: The facility shall have an emergency plan that provides for accommodations for shelter of children during an emergency including lock-down, shelter in place at the facility and shelter at locations away from the facility premises. The facility shall have an emergency plan that provides for accommodations for evacuation of children from the facility building and evacuation of children to a location away from the facility premises. The evacuation routes and evacuation plans to exit the building may be the same as those required by §3270.94 (relating to fire drills).

Noncompliance Area: During the renewal inspection, certification representative did a facility walk through and noticed an individual,who was later identified as the owners husband, was in the facility interacting with children and going in and out of rooms during naptime. Owner was asked if her husband had a file on site and he did not have a file or clearances. Certification representatives understanding is that the husband is in charge of maintenance and doesn't hold a position working with the children despite being allowed to interact with them.(HUSBAND left the facility during the renewal inspection)

SAFETY PLAN: The operator is required to submit to the Children and Youth Agency a plan of supervision or alternate arrangement as required by the Child Protective Services Law. LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 5 may not work in a child care position at the facility. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #5- may not work in a child care position at the facility.

A facility person's record shall include verification of child care experience, education and training prior to service at the facility.

Noncompliance Area: Provider has not been taking children outdoors at all when in care at facility.

Correction Required: Weather permitting, children shall be taken out doors daily.

Description: Mixed Age Level

Noncompliance Area: During the inspection the staff person that answered the door of the facility left their primary care group and left the room with a 2:18 staff child ratio, including one-year olds. The two-year old room was observed at the time of the inspection with a 1:12 staff to child ratio.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level).
