Description: Based on staff interview, on 10/15/2019 child #2 was bitten on the wrist while in the bucket table, leaving a red mark. Staff A witnessed this and staff F reported this to the parent at pick up time the same day, however this injury was not documented in the facility medical log.

Description: Based on observation, record review and interview, documentation of staff hours in-ratio was not complete and accurate, as follows: Staff D was observed caring for children in the "main room" upon my arrival, however this staff person was not signed in. Staff G was signed into the "main room" upon my arrival, however this staff was observed now caring for children in the infant room, but was not signed into the infant room.
