Inspection Date Reason Description Status
2025-11-24 Renewal 3270.124(b)(1) - Child's name, birth date Compliant - Finalized

Regulation: 3270.124(b)(1)

Description: Child's name, birth date

Noncompliance Area: The emergency contact form on file for child #1 did not specify the child's birthdate.

Correction Required: Emergency contact information must include the name and birth date of the child.

Provider Response: (Contact the State Licensing Office for more information.)
The birthdate area was filled out by the parent; however, the parent originally put their birthdate. Following this discovery, we noticed this was corrected on forms in all staff binders and there is a second sheet with the correction in child #1's file. (See attachment)
2025-11-24 Renewal 3270.124(c) - Each child care space Compliant - Finalized

Regulation: 3270.124(c)

Description: Each child care space

Noncompliance Area: There was no emergency contact form present in Room 2 for one of the children receiving care in that room.

Correction Required: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space.

Provider Response: (Contact the State Licensing Office for more information.)
Child #1 was a late enrollment and his ECF was distributed to each staff member and the change to the birthdate was communicated. The AGS(Executive Director) was serving in the room until the inspector came. The AGS, who had the ECF in her binder, asked another staff member who serves the children in that room regularly to fill in during the inspection, however that person never added the ECF to her binder when it was distributed. The AGS presented the ECF to the inspector but was not working in the room at the time due to the inspection. The staff person was given another copy of the child's ECF and it was placed properly in her binder.

Correction Required: facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Cleaning and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Provider Response: (Contact the State Licensing Office for more information.)
The inspector determined the hand sanitizer in a child off-limits area between the two rooms, used by staff only, was non-compliant. The doors to this area were immediately locked. Also, hand sanitizer was not visible but discovered when a white board was removed by the inspector. Because all hand sanitizers were previously removed from rooms 1 and 2, this was not noticed during the daily facility check. They were immediately removed when discovered. The hand sanitizers from Room 2 were removed. Furthermore, the doors to the upstairs classroom and second floor of the facility are closed due to lack of use during the fall session. In the large upstairs meeting room, a bottle of hand sanitizer was observed on an enclosed area desk, and a bottle of disinfectant spray was discovered on a desk shelf area. All named toxins were immediately removed from the second-floor meeting room/classroom.

Provider Response: (Contact the State Licensing Office for more information.)
Staff members 2 and #5 were always supervised immediately following the inspection as they awaited the upcoming CPR/First Aid training. Due to rising costs and staff accessibility issues associated with free training along with the impact on the SHCDP 2025 budget, paid CPR training sessions will be hosted by SHCDP twice a year. SHCDP Facility person #2 is a new hire and the AGS(Executive Director) was in the room until the inspector came. The AGS asked another person to fill in but that person ended up leaving the position during the inspection. Facility person #5 just returned to the organization for the fall programming. Both people are scheduled for training during our next session on January 14, 2026. We understand that Facility persons #2 and #5 must be supervised if they interact with children; and if no qualified staff person is available to supervise, Persons #2 and #5 may not work inside the facility nor transport them.

Facility person #5 went to [their] regularly scheduled doctor's appointment in December. [They] forgot the form to have the physician fill out. He called the office and they promised to send it to him after the holidays. To ensure his biennial assessment is completed in a timely manner, we again requested a completed form from the physician. We also spoke with said Facility person and staff during the annual January staff meeting and reminded them that they cannot wait until the last minute to renew certifications. They need to book appointments one month in advance to in this case, coincide with health insurance plans and the number of times they can have check-ups during the coverage year.

Provider Response: (Contact the State Licensing Office for more information.)
As explained in a prior inspections, SHCDP receives food from an external resource, a government agency that relies on donations and menus are distributed within the week before the first of the month. We continue to request the menus when we do not have them in a timely manner to meet the regulation and receive them accordingly. Admittedly, since this violation has been brought to the attention of the food providers, we have received the menus earlier during recent months than in the past. In this case, the Thanksgiving holiday began the next day. We received the December menu upon return to the program. The menu was emailed the day following the inspection.

All emergency phone numbers are posted on the outside wall of this phone location in each room and corridor. This is cordless phone on a charger that is often used outside of the kitchen. We have never been cited for this phone location. Nonetheless, a list of emergency phone numbers was placed in the kitchen at the phone charger's location.
2025-11-24 Renewal 3270.76 - Building Surfaces Compliant - Finalized

Regulation: 3270.76

Description: Building Surfaces

Noncompliance Area: There was an exposed bolt in the outdoor playspace at the bottom of the gate.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Provider Response: (Contact the State Licensing Office for more information.)
The covering for the bottom bolt was pulled off and no longer fully covered the bottom ground-level bolt like the bolts above it. Due to weather, this area is not used as much during the months of November-March. All the bolt coverings were recently replaced, and intact as observed during the inspection. However, the very bottom one was probably the result of deer and rabbits which often try to gain access through this entry gate of the fence. The covering was replaced the next morning after discovery on 11/25/2025.
2025-11-24 Renewal 3270.94(a)(1) - Every 60 days Compliant - Finalized

Regulation: 3270.94(a)(1)

In this case, the fire drill would have been conducted on 10/24; however, we were not in session that day (Fri. 10/24). We conducted the fire drill the next day children were present (Mon. 10/27). We understand the regulation that fire drills must be completed at least every 60 days and need to include children in attendance. We have reviewed the calendar, adjusted the Fire drill dates, and noted these dates on our calendars.
2025-07-30 Allocated Unannounced Monitoring 3270.102(a) - Clean and good repair Compliant - Finalized

Regulation: 3270.102(a)

Description: Clean and good repair

Noncompliance Area: There was a pencil box in Room 1 that was cracked, creating sharp edges/a potential pinch point.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Provider Response: (Contact the State Licensing Office for more information.)
A plastic pencil box was observed in Room 1 with a crack that determined to be a potential pinch point. This occurred due to routine use. Each child or small group has their own pencil case. SHCDP observes the condition of its classrooms, toys, supplies on a daily basis and while we observed nothing amiss with the other 13 boxes, we missed this particular pencil box in question. The box was removed and replaced. Staff were re-trained during the school year orientation on August 16th to monitor toys, desks, supplies, pencil boxes, etc. to ensure there are no hazards to children.
2025-07-30 Allocated Unannounced Monitoring 3270.133(4) - Locked Compliant - Finalized

Regulation: 3270.133(4)

Description: Locked

Noncompliance Area: There was an inhaler accessible to children in a backpack in a cubby in Room 1.

Correction Required: Medication shall be stored in a locked area of the facility or in an area that is out of the reach of children.

Provider Response: (Contact the State Licensing Office for more information.)
We learned that unbeknownst to us, the mother placed that inhaler inside for the father (who had custody of the other child) to pick up for that sibling who needed it later that day. This is a shared custody situation. The bag belongs to a child who does not use an inhaler! The inhaler was removed and placed inside the medicine cabinet. We spoke with the parent and reminded all parents that SHCDP must be notified of any medications brought into the facility.
2025-07-30 Allocated Unannounced Monitoring 3270.133(6) - Written consent Compliant - Finalized

Regulation: 3270.133(6)

Description: Written consent

Noncompliance Area: There was no written consent from the parent to administer the following medications at the facility, labeled for the use of child #1: olapatadine, fluticasone, ceterizine, and an epipen.

Correction Required: A parent shall provide written consent for administration of medication or a special diet.

Provider Response: (Contact the State Licensing Office for more information.)
Apparently, our current written consent/signatures from parents to administer medications in our possession to the children is no longer adequate. For years, parents have signed permission for administering medications on page 3 of our applications. The parent of child #1 signed the part of the application that allows for this. During the inspection, medications for children were observed as labeled with child's name and the name of the medication, instructions for use, a general log is available in the event SHCDP staff had to administer the medication.
2025-07-30 Allocated Unannounced Monitoring 3270.14/3270.21 - Pertinent Laws & Regulations/General Health and Safety Compliant - Finalized

Regulation: 3270.14/3270.21

Description: Pertinent Laws & Regulations/General Health and Safety

Noncompliance Area: Facility Person #4 was supervising a group of children along with two volunteers (Facility Persons #2 and #3) in Room 2. There was no documentation on file indicating that Facility Person #4 has completed the required pre-service trainings (pediatric first aid & CPR, mandated reporter training, health & safety training.)

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Until such time as the required pre- service trainings are completed, staff person #4 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #4, staff person #4 may not work in a child-care position at the facility.

Provider Response: (Contact the State Licensing Office for more information.)
Facility person #4 no longer works for SHCDP. At the time of inspection on the next to last day of our six-week summer camp, Facility person #4 was observed in Room 2 with two volunteers as the children were finalizing their presentations and preparing to go to the park and in turn, freeing the space for other staff to move items from the storage closet to outdoors for our annual last day outdoor celebration. The staff member with all certifications and designated to supervise #4 was nearby moving lunches and camp bags for transport. We learned that the disruption of the inspection was intimidating to the supervising staff member who opted to move out of the way of the two inspectors. The outing was cancelled due to the inspection. All staff were retrained on supervision and how to deal with inspectors moving about.
2025-07-30 Allocated Unannounced Monitoring 3270.14/3270.21 - Pertinent Laws & Regulations/General Health and Safety Compliant - Finalized

Regulation: 3270.14/3270.21

Description: Pertinent Laws & Regulations/General Health and Safety

Noncompliance Area: The following toxics were observed accessible to children: ice melt on the floor of an unlocked cabinet in Room 2. lotions in bags in cubbies in Rooms 1 & 2, and on a table in Room 2, cleaning supplies in an unlocked low cabinet in the unlocked kitchen.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children.Cleaning and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Provider Response: (Contact the State Licensing Office for more information.)
The inspector determined toxins were accessible to children on the next to last day of summer camp. Prior to the inspector's arrival, children were preparing for an outing, and the timing of their departure was supposed to coincide with other staff members bringing water hoses and wood display boards, etc. from the always locked storage room. The storage closet was unlocked in anticipation of the entire group from rooms one and two being gone from the facility. Ice melt was moved to the front to allow access to dolly's located in the back. The door was immediately locked when the children's outing had to be cancelled. Regarding the lotions, Staff was gathering their bags for departure and the person designated to lead the children onto the vans placed her bag on a shelf in preparation for departure. The inspector went inside her bag that had undergarments and a shirt on top and found lotion. The staffer's bag was immediately moved to a secure area where it would be out of the reach of children. In the same manner the inspector proceeded to go through bags in cubbies. Because this was a designated park (sprinkler) day, as part of skin care and hygiene needs parents provided lotions and soaps for their children in their camp bags which were stored in cubbies away from where children were gathered in the rooms. The inspector went through the bags and discovered the items among towels and other water activity gear. We removed all lotions, soaps, and sunscreens from the bags as advised. The kitchenette has had these designed childcare doors (half-doors) for many years, which secure the area. There is no handle or way for children to grab the door and open it. Children cannot gain access inside the kitchenette area from the outside. See picture. Entry doors to the kitchenette are always closed. Cleaners were placed in the cabinet as staff was cleaning equipment and preparing for the outdoor Celebration. Upon the advisement of the inspector, locks were placed on the doors and cleaners were moved to a secure cabinet. These actions will be reviewed during future staff training to make sure cleaning and other toxic hygiene items such as lotions are kept in an area inaccessible to children.
2025-07-30 Allocated Unannounced Monitoring 3270.151(a)/3270.192(3) - 12 months prior to service and every 24 months thereafter/Health assessment, TB test Compliant - Finalized

Regulation: 3270.151(a)/3270.192(3)

Description: 12 months prior to service and every 24 months thereafter/Health assessment, TB test

Noncompliance Area: The health assessments on file for Facility Persons #1 and #4 were each completed more than 12 months prior to their respective start dates. There were no health assessments/TB test results on file for Facility Persons #2, #3 (volunteers who have contact with children).

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Provider Response: (Contact the State Licensing Office for more information.)
Facility Persons #2 and #3 no longer work for SHCDP. They were applicants and delayed in getting their health assessments/tb reading due to transportation issues and appointment scheduling with their physicians. As explained to the inspector, Facility Person #1 was not working in the facility at the time of inspection. He had just come back from his professional football season, and he was offering to assist with putting up tents and building a stage and boards, etc. for the kids displays for the next day's celebration. As the inspector witnessed, he said goodbye to me and left the building during the inspection. Also, he mostly works during the school year until his football training season begins. His health assessment was valid before he left the country and we knew that if he were to come back for the school year, he would have to get a new health assessment performed, which he did on August 27th for a September start back in the facility. See attachment.
2025-07-30 Allocated Unannounced Monitoring 3270.161(d) - Potentially hazardous food refrigerated Compliant - Finalized

Regulation: 3270.161(d)

Description: Potentially hazardous food refrigerated

Noncompliance Area: There were two children's lunches in cubbies in Room 2 containing grilled cheese sandwiches.

Correction Required: Potentially hazardous food brought from the child's home or provided by the facility shall be refrigerated.

Provider Response: (Contact the State Licensing Office for more information.)
As explained to the inspectors and in the above POC's, the day of this inspection was a busy one because it was the day before the end of camp and to coordinate preparing the facility for the Camp Celebration the next day, the plan was for the children to not be in the facility so we could move equipment, begin setting up tents, chairs, tables, and building display boards for the kids' work, etc. The food from the refrigerator was moved to the kids' cubbies for the outing prior to the arrival of inspectors and were to be placed with the group's bag and room cooler. The children were finishing up writing their final camp thoughts while staff were getting prepared for the outing. The "to-go" cooler with the provider lunches was ready to go but due to the inspection, the "from home" lunches were not placed inside yet. This is not the normal process usually it's done at the same time.
2025-07-30 Allocated Unannounced Monitoring 3270.192(5) - Two written references Compliant - Finalized

Regulation: 3270.192(5)

Noncompliance Area: There were no written nonfamily references on file for Facility Persons # 2, #3, and #4.

Correction Required: A facility person's record shall include two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person.

Provider Response: (Contact the State Licensing Office for more information.)
Facility Person #2, #3, and #4 no longer work for SHCDP. These people were seasonal (5-6 weeks) employees. Their references were not received during the short time frame to place in their files.
2025-07-30 Allocated Unannounced Monitoring 3270.21 - General Health and Safety Compliant - Finalized

Regulation: 3270.21

Description: General Health and Safety

Noncompliance Area: Smoke detector testing was documented as completed on 5/2/25, then again on 6/2/25. There was no documentation of smoke detector testing after 6/2/25. Smoke detectors are required to be tested, and the testing documented, at least every 30 days.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children. School Age Child Care programs that are not in a school building must comply with the fire safety laws contained in the Human Services Code, 62 P.S. § 1016(c) and 34 Pa. Code §403.23.

Provider Response: (Contact the State Licensing Office for more information.)
As observed, Smoke detector testing was documented as completed on 5/2/25 and then on 6/2/25. However, the signature on 6/2 was omitted because another sheet was being prepared for the new year in preparation for our annual independent Fire system inspection conducted by PFE/Cintas and the required emergency letters to County EMA officials. Facility persons #2 and #3 were removed and no longer work for SHCDP. Facility persons #2 and #4, seasonal volunteers, had not received the results of their FBI and NSOR clearances. This was due to the timing of their being in the facility and the process. Facility Persons #2 and #4 were removed from inside the facility and no longer work for SHCDP.

Facility person #4 is a tenured, certified, and qualified School District teacher. Her documentation was presented electronically. This was not presented to the inspector due to the disruption of the day as we were preparing to pick up supplies and move as planned in preparation for the next day's celebration.

Exposed bolts on the entry gate of the fenced-in area were observed. The covering was there before the start of the summer program as observed in previous inspections. The exposed bolts were the result of weather damaging the covering and normal summer wear and tear. We typically replace covering and make fence repairs during the transition and facility repair period that occurs for the Celebration and the start of the school year. The covering was immediately replaced. See attachment.
2025-07-30 Allocated Unannounced Monitoring 3270.94(a)(1) - Every 60 days Compliant - Finalized

This violation is the same as noted in Violation 3. The fire drill did occur. The posted document was not current. As observed, Smoke detector testing was documented as completed on 5/2/25 and then on 6/2/25. However, the signature on 6/2 was omitted because another current sheet was being used along with the current fire drill documentation in preparation for the new year and our annual independent Fire system inspection conducted by PFE/Cintas and the required emergency letters to County EMA officials. The current completed sheet which contains documentation of the fire drills was in a file to present to the independent fire alarm and equipment inspectors, who were scheduled to conduct the inspection a couple of days later. The testing was actually completed and fire drill completed but the documentation was on another sheet that was not posted. This was immediately corrected by the Executive Director by making copies for the Fire system inspectors and placing the documentation where it was originally posted.

Provider Response: (Contact the State Licensing Office for more information.)
SHCDP will review its options to resolve this matter. This was a customized vehicle. The vehicle was purchased in December 2019. Prior to purchase we called OCDEL to confirm this vehicle was acceptable because it was customized. At the time, the response was that there was nothing in the code to prohibit customization and the FAQ sheet at the time did not address it. After much time spent on this, we have not been questioned about the 10-seater since, and we have never been cited for it by inspectors over the last five years. The documentation that approved this vehicle by inspectors is with licensing inspection reps. After five+ years of inspections and observations by licensing inspectors, this new determination is challenging as it may involve substantial costs, a great loss, reducing the number of children we serve, and could impact staff employment if we have to get another CDL driver for another large bus.
