In space #1B, one (1) aerosol can of Micro-Ban was stored on the shelf above the desk used by the teacher. In the "girls" bathroom used by children one (1) year of age and older, two (2) aerosol cans of Lysol were stored in an unlocked white cabinet on the wall to the left of the diaper changing table.

In the "girls" bathroom used by children one (1) year of age and older, one (1) container of baby wipes stored in the plastic container labeled with warnings in addition to "Keep out of Reach of Children" was stored on the diaper changing table less than five feet from ground level and accessible to children under three (3) years of age.

A safe indoor and outdoor environment was not provided for the children. The handicap accessible ramp leading out of space #4, has a portion that has collapsed. The handicap accessible ramp leading out of space #5, has two (2) planks that are broken and have splintering wood and exposed nails. A hole six (6) inches deep was near the fence and extending under the concrete on the playground used by infants.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Five (5) electrical outlets next to the water fountain between the bathrooms used by children enrolled were not covered with a safety cover. In space #5, two (2) electrical outlets were not covered in the block area, one (1) electrical outlet was not covered under the television, one (1) outlet was not covered in the art area, two (2) outlets were not covered near the easel and one (1) outlet was not covered in the area used to play board games.
