All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In the children's dining area of Building #2, there was an unlocked cabinet below the sink with an aerosol can of snow spray. In the same space in a cabinet with a lock that was not latched closed, there was an aerosol can of Air Wick odor neutralization, an aerosol can of True Living air fresher spray, one bottle of Clorox urine remover spray, one aerosol can of Scotchgard fabric protector, one aerosol can of Sure Scents automatic spray refill, one bottle of Fabuloso multi-purpose cleaner, one container of Spartan F-6 flying insect killer and one bottle of Scotchgard fabric cleaner. In the same space, there was an unlocked cabinet by the food storage that had a D-Con Mouse Trap, with multiple warnings. In the bathroom beside the kitchen of Building #1, there were two aerosol containers of Febreeze room spray and one aerosol container of Wizard air freshener hanging on the wall, one container of Clorox bleach and one spray bottle of Fabuloso on a shelf beside the toilet. In Building #1 Space 1D, there was an unlocked cabinet above a crib with an aerosol can of Sure Scents air fresher.

Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. An incident report completed on 2/15/23, for a one year old child, did not have the information of the date/time that the parent was contacted and by whom.

Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In Building 2, Space 1, where a two year old child was in care, a Wal-Mart plastic grocery bag with safety warnings was on a shelf in the art center. In the same space there were multiple broken pieces of crayons approximately one half to one inch in length, 12 foam rubber stamps, 10 foam sponges and an opened package of small red/green craft pom poms.

Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. One half full cup of yellow colored liquid with a straw was sitting in the art activity area accessible to children in Building 2 Space 1.

My son was here for two weeks. When he would get home he would be starving because they feed them at 11pm and then nothing but a small snack at 2pm which Im not sure they even gave him. So I took it upon myself to buy snacks and send him juices and milk so that he would not be hungry, and the lady "Peggy" that runs the center took the food from my son and refused to give it to him. This is when I pulled him out because it is ridiculous that I am paying 135$ a week plus buying snacks to make sure he is eating and then he gets home and is acting like he hasn't eaten all day!
