Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: On 08/27/2020, observed a preschool age child sleeping on a blanket on the floor in lieu of age-appropriate rest equipment.

Correction Required: Bed linens may not be used alone as age-appropriate rest equipment.

Noncompliance Area: On 09/02/2020, the file for child #1, a 2-year-old, included a record of the child's immunization status which lacked an influenza immunization.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: On 08/27/2020, observed the staff health assessment for staff person #1 was not completed at least once every 24 months; the file contained a current health assessment dated 08/21/2020, with the previous health assessment dated 10/31/2017.

Correction Required: A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: On 08/27/2020, the lunch prepared at the facility lacked at least one protein; the lunch served that day included: a grilled cheese sandwich, apple sauce, potato chips and apple juice.

Correction Required: A lunch or dinner prepared at the facility for children of toddler age or older shall include at least one protein.

Noncompliance Area: On 08/27/2020, observed electrical outlets on a power strip which lacked protective receptacle covers on the outlets not in use; the power strip was located beside the TV in the living room. Observed an electrical outlet in the hallway leading which lacked a protective receptacle cover.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: On 08/27/2020, observed a broken child safety latch on the cabinet under the bathroom sink and under the kitchen sink allowing access to toxic materials. The bathroom cabinet contained Lysol spray, cleaning products, hair products and bath products labeled keep out of reach of children. The kitchen cabinet contained Lysol spray cleaner, lotions and cleaning products labeled keep out of reach of children.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: On 08/27/2020, observed the key to the cabinet where firearms are contained was accessible to the children in care; allowing access to the firearms. The cabinet was locked with the key inside the keyhole. An older toddler was napping in a Pack 'N Play in the room the cabinet is kept.

Correction Required: If the facility is located in a residence, weapons and firearms shall be contained in a locked cabinet.

Noncompliance Area: The file for child #1 lacked a child service report with the services to be provided to the family and the child at least once every 6 months; the file contains a child service report dated 08/11/2018 and 02/09/2018 with no current child service report on file.

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at 3290.124(e).

Noncompliance Area: The file for child #1 lacked an updated health report at least once very 6 months for a young toddler; the file contains a current health report dated 06/11/2019 and a previous health report dated 08/07/2018 with so subsequent health report in between.

Correction Required: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler

Noncompliance Area: The file for child #1, a two year old, included a record of the child's immunization status which lacked a 2nd dose of Hep A immunization.

Correction Required: The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP.

Noncompliance Area: Fire drills were not conducted at least four times per year; fire drills were documented on 08/15/2018, 02/18/2019 and 08/08/2019.

Noncompliance Area: On the date of the inspection, the operator was caring for another family members dog "Sadie". The operator did not have a copy of Sadie's rabies immunization.

The file for child #2, with an enrollment date of 01/31/2018 has a child health report dated 08/07/2018 with no additional health reports on file.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: Observed a tube of diaper rash cream on a hutch located in the kitchen which was accessible to children in care. Observed a variety of prescription and non-prescription medications including: ibuprofen, pain relievers, antibiotic gel, cold medications and Pepto-Bismol in an unlocked medicine cabinet above the sink located in the bathroom used by the children in care. Observed multiple boxes of Sentiel heartworm medication on the microwave stand located in the dining room which were accessible to children. Observed prescription medications in an unlocked drawer next to the sink located in the kitchen which were accessible to children.

Correction Required: Medication shall be stored in a locked area of the facility or in an area that is out of the reach of children.

Noncompliance Area: Observed kitchen knives and adult scissors in an unlocked drawer located in the kitchen which were accessible to children. Observed matches and a lighter in an unlocked drawer on the microwave stand located in the dining room which were accessible to children.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Observed hair gel and deodorant in an unlocked drawer in the vanity located under the sink in the bathroom. Observed lotions and mail polish small cabinet located in the bathroom.

Noncompliance Area: The sliding glass door leading from the living room to the outdoor play space lacked a visual strip or other visual identification on the glass.

Noncompliance Area: The facility lacked documentation of fire drills at least four times per year, on the date of the inspection, the operator was unable to locate the fire drill log.

Noncompliance Area: The outdoor play space was observed having exposed bolts at the corners and entrance/exits of the chain link fencing surrounding the play space.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: The living room used as the main child care space was observed having a cabinet with items having a diameter under 1 inch accessible to all ages of children.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: The household dog named "Hillary" lacked a current rabies imunization.
