Please remember to review, update the information, if necessary, and initial and date the review on an annual basis. Provider reviewed, initialed and dated her emergency disaster plan on-site. Findings: During the inspection, the floor temperature in the front area of the child care space only reached 60 degrees F. When the thermometer was moved into the playroom area on the carpet, the temperature on the thermometer reached 65 degrees F. Despite the heater being turned on, the floor temperature in the front area never went over 60 degrees F during the course of the inspection. The floor temperature in the child care space cannot be less that 65 degrees F. Please write a letter of correction stating how you will become in compliance with this regulation in the future.

Findings: During a review of emergency cards, it was revealed that some information was missing. LC is missing a contact phone number for one of his emergency contacts. HA is missing all of his physician's information. NC is missing her doctor's phone number. Please inform the parents of the missing information and have them add it to their respective emergency cards, initial and date the addition of the information. Provide a picture of the updated emergency cards to OCC for the correction of this non-compliance.
