Findings: During a review of children's records, it was found that one 2 year old child does not have Health Inventory Part II in his file. He is also missing another DTP immunization for his age. A one year old child is missing an additional 2 DTP immunizations, an additional 2 Polio immunizations, an additional Hep B immunization and an additional Prevnar immunization for her age. Her sibling, a 5 year old child is missing an additional MMR immunization. That same 5 year old child's parent reports taking a religious exemption for the blood lead testing on the blood lead testing form, but the physician must also sign that form. Please inform the respective parents of the missing information for their children so that they might obtain the information, supply it for their child's file and send a copy of the information to OCC for the correction of this non-compliance.

Findings: Upon inspection, the Provider's First Aid Kit was missing one out of two flexible rolls of gauze, paper towels, a thermometer and a triangular bandage. During the inspection, the Provider was able to add paper towels and a thermometer. Therefore, the Provider only needs to add one roll of gauze and a triangular bandage. Please do so and send a picture of these items to OCC for the correction of this non-compliance

Findings: The family began fostering a dog in November of 2023 and then officially adopted her in December of 2023. OCC was not made aware of Brownie until today's unannounced visit. The Provider is reminded that OCC is to be immediately notified of any changes in the home that may present a risk to the health, safety or welfare of the children in care. Regulation was previously discussed with Provider at the last inspection regarding another dog in the same situation.

Findings: During the inspection, the cabinet under the kitchen sink was unsecured and contains cleaning agents that are potentially harmful. During the inspection, Magnesium tablets were found in a drawer in the bathroom that the children use. These were also unsecured. The container says "keep out of the reach of children". When this was brought to the attention of the Provider, she immediately corrected the Magnesium tablets. Before the Licensing Specialist brought the unsecured kitchen cabinet to her attention, she secured it herself. Therefore, both were corrected on-site.

Findings: During a review of children's emergency cards, it was revealed that child FF has the wrong year of birth listed on her emergency card. Additionally, FF does not have any physician information included on her emergency card. FF's father arrived during the inspection. The Provider instructed the father to amend the emergency card; adding the correct date of birth and the physician's information on her emergency card. Therefore, this non-compliance was corrected on-site.
