| Findings: During a review of children's files, it was found that a five year old boy, who was admitted into care on 06/09/2026 and present during today's inspection, is missing Health Inventory Part I and Part II as well as an immunization record. He is also missing any evidence of ever having a blood lead test. Please obtain the aforementioned documents for the identified child and place them in his file and send a copy of the documents to OCC for the correction of this non-compliance. | |||

| Findings: During a review of children's files, it was found that an 18 month old girl is missing her 12 month old blood lead test as well as age-appropriate immunizations. She is missing another DTAP, an MMR and a Varicella vaccination. Please inform the parent of the need for the blood lead test so that the parent can arrange testing with the physician and submit test results for her file. Notification in writing of this need to the parent and copied to OCC will suffice as the correction for the blood lead test portion of this non-compliance. Please also notify the parent of the need for an updated immunization record to include the missing age-appropriate immunizations. When received, add the age-appropriate immunization record to her file and send a copy of the updated immunization record to OCC for the correction of this non-compliance. | |||

| Findings: The playset in the outdoor play area has several entrapment issues. When discussed with the applicant and her husband, they both reported that they could easily eliminate the entrapment areas. Please eliminate the entrapment issues on the playset and send a picture to OCC to show this modification. In addition, dog feces was observed in the play yard area. Please ensure that there is no animal feces where children will be. Please write a letter of correction for this portion of the non-compliance. | |||
