The legal entity must submit documentation in a format approved by DHS proving that the facility is maintaining the required supervision groups at all times. The Regional Office will provide supervision forms for facility director to complete and document supervision groups for a period of three months. Documentation will remain at the facility and will be checked by a Regional Office Representative during monthly unannounced visits. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. A lock has been placed on the playground gate and can only be opened with a key that is kept by staff. If classes are on the playground during pick up time, staff will unlock the gate when parents arrive to pick up children and then staff will lock the gate again after a child is picked up.
