Noncompliance Area: At time of unannounced inspection on 9/18/25, an 8-month-old infant was observed sleeping in a small rocking seat that was placed on the floor.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Noncompliance Area: At time of unannounced inspection on 9/18/25, an infant was observed sleeping in the same group space where pre-school aged children were working on small muscle activities.

Correction Required: Preschool and school-age children may not be involved in small or large muscle activity in the same group space in which children are sleeping or resting.

Noncompliance Area: At time of renewal inspection on 8/20/25, plastic bags (diaper bags/grocery bags) were observed in cubbies in the middle room and were accessible to children under 3 years of age.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: At time of renewal inspection on 8/20/25, three children (infants/young toddlers) were observed to be in highchairs for at least 50 minutes and were not eating, while other children in the classroom were at the table working/playing.

Correction Required: Infants and toddlers shall be provided stimulation by being held, rocked, talked to, played with and carried. A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs.

Noncompliance Area: At time of renewal inspection on 8/20/25, children's packed lunches brought from home containing items such as pepperoni and cheese were observed on top of the cubbies in the middle room and were not refrigerated.

Correction Required: Potentially hazardous food brought from the child's home or provided by the facility shall be refrigerated.

Noncompliance Area: At time of renewal inspection on 8/20/25, hand sanitizer was observed on a shelf in the Pre-School/Pre/K room and was accessible to children. Diaper cream labeled "keep out of reach of children" was observed in the unlocked changing table in the Pre-School bathroom and was accessible to children.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: At time of renewal inspection on 8/20/25, toxic Vinca flowers were observed hanging on the fence surrounding the outdoor play space used by the children and were accessible to children.

Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: During initial inspection 05/14/2024, Certificate of Occupancy and Food Prep license were not posted in facility.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation prior to issuance of Certificate of Compliance. A certificate of compliance will not be granted by the Department until the legal entity provides a certificate of occupancy as proof of compliance with the applicable requirements of the Department of Labor and Industry in 34 Pa. Code § 403.23 (relating to child day care facilities) and under section 1016(c) of the act (62 P.S. § 1016(c)).

Noncompliance Area: During initial inspection on 05/14/2024, the following toxic plants were observed in the front room of the child care facility: Monstera, Golden Pothos; the following toxic plants were observed in the rear room of the child care facility: Rare Alocasia Amazonica, Boston fern, Blushing Philodendron.

Correction Required: Toxic plants are not permitted in a child care space prior to issuance of Certificate of Compliance.

Description: Building Surfaces/Clean, good repair, proper size

Noncompliance Area: During initial inspection 05/14/2024, two bookcases in rear room of facility were not mounted to the wall posing a potential tipping hazard.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs prior to issuance of Certificate of Complaince.

Noncompliance Area: Upon review of the immunization record of facility child #1, Certification Representative noted the child was not up to date with immunizations as recommended by the ACIP. There was not documentation in the record from the health provider regarding a plan to provide ongoing vaccines per the recommended schedule.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: Certification Representative observed facility child # 1, who is an infant was sleeping in equipment that was not appropriate rest equipment. Staff acknowledged that the child had fallen asleep and they had not had a chance to move the child to a cot. Certification Representative observed that cot chart and noted facility child #1 was not listed on the chart nor was any equipment labeled with the child's name.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: Certification Representative observed 11 children, including 3 infants, 1 young toddler, 2 older toddlers and 5 preschoolers in a classroom with 2 staff. The required ratio for this mixed age group should have been 3 staff.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in 3270.51 (relating to similar age level).

Noncompliance Area: CHILD 1 AND CHILD 2 DID NOT HAVE AN UPDATED EMERGENCY CONTACT FORM (LAST UPDATE 12/29/2017 AND 12/27/2017).

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the informationA parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: STAFF 1 - 3 DID NOT HAVE A CURRENT FIRE SAFETY TRAINING - LESS THAN A YEAR OLD - LAST ON DATED 5/19/17.

Correction Required: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Noncompliance Area: THE PAINT ON THE PURPLE HALF WALL WAS CHIPPED.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: THE VAN HAD A CAPACITY OF 7 AND THERE WERE 2 STAFF AND ATLEAST 7 CHILDREN IN THE VAN. THERE WAS ALSO ONLY 5 CAR SEAT IN THE VAN FOR THE 7 CHILDREN.

Correction Required: A child 7 years of age or younger shall be transported in accordance with the requirements for parents and guardians as stated in 75 Pa.C.S. 4581 (relating to restraint systems). Safety restraints installed in the vehicle at the time of manufacturing shall be used by all occupants.

Noncompliance Area: STAFF 1 DID NOT HAVE THEIR STATE POLICE CLEARNCE ON FILE AND THEIR START DATE WAS 7/10/17.

Noncompliance Area: STAFF #1 HEALTH REPORT FORM WAS MISSING THE CHECKS FOR THE BOXES THAT STATE IF THE STAFF IS FREE OF COMMUNICABLE DISEASES AND IF STAFF IS PROHIBITTED FROM PROVIDING ADEQUATE CARE TO CHILDREN.

Correction Required: An adult health assessment must include an examination for communicable diseases and the results of that examination. An adult health assessment must include information on medical problems that might threaten the health of the children or prohibit a staff person from providing adequate care to children.

Noncompliance Area: STAFF #1 DID NOT HAVE THE PROOF OF EXPERIENCE IN THEIR FILE FOR THE POSITION THAT WAS STATED THEY HELD.

Correction Required: A facility person`s record shall include verification of child care experience, education and training prior to service at the facility.
