|Findings: Based on the Surveyor's observations, the medication for child #1 was an old medication that expired in November 2016 and had not been returned to the enrolled child's parent. * Repeat deficiency from the 11-15-16 Compliance inspection.|
|Findings: Based on the Surveyor's observations, there were 2 containers of glass cleaner in 2 different unlocked cabinets located below the diaper changing counter accessible to enrolled children in the toddlers' classroom. * Repeat deficiency from the 11-15-16 Compliance inspection.|

|Findings: Based on facility documentation and the Surveyor's observations, it was determined that the file for staff #1 lacked documentation of a fingerprint clearance card (Date of employment-08/03/16). The fingerprint clearance card for staff #2 was not verified upon employment (Date of employment- 10/11/16). The fingerprint clearance card application for staff #3 was not submitted to DPS within 7 working days of employment (Date of employment- 04/11/16, date application was received by DPS- 05/10/16). The fingerprint clearance card application for staff #4 was not submitted to DPS within 7 working days of employment (Date of employment-09/28/15, date application was received by DPS- 10/22/15).|
|Findings: Based on the Surveyor's observations, there was 1 uncovered electrical outlet on the south wall under the sign in/out desk in the Pre-K classroom.|
