Findings: Based on the Surveyors' observations and staff interview, it was determined that the main posting board lacked the name of the current facility director and the name of the individual designated to act on behalf of the facility director when the facility director is not present in the facility. The Surveyors were informed that staff #2 was in charge, however, staff #2's name was not listed on the main posting board.

Findings: Based on facility documentation, the Surveyors' observations, and staff interview, it was determined that 1 out of 10 enrolled children's attendance forms was incomplete and lacked the following required information: Child #3: An admission signature for 02/11/2020. Staff stated the parent forgot to sign their child in that morning.

Findings: Per allegation and based on the Surveyors' observations, facility documentation, and staff interview, it was determined that the licensee failed to ensure that the health, safety, or welfare of an enrolled child was not placed at risk of harm. The licensee allowed 2 enrolled children with severe behavioral issues to remain in the classroom with other enrolled children. Because these children were allowed to remain, other enrolled children and staff have sustained injuries due to the children's actions. Upon the Surveyors' arrival into classroom 1E on 02/11/2020, Surveyor #2 was hit with toys that were flying through the air thrown by child #1. Surveyor #1 arrived in the room seconds later, and the following events took place during the investigation: * Child #1 ran across the room and as a staff member attempted to calm the child, the child hit the staff member in the face. * Child #1 jumped up on top of a table, threw Legos across the room, and jumped off the table. * Child #1 ran across the room and threw a chair. * Child #1 jumped off a chair and onto the restroom half-door. * Child #1 ran from a staff member to the opposite side of the room. * Child #2 began to throw toys at a staff member and as the staff member tried to pick child #2 up, the child fell on the floor and began to scream loudly. * Surveyor #1 quickly went to get an additional staff member, whom she was told was in charge of the facility at the time. That staff member came into the room and attempted to calm child #1 down as another staff member tried to quiet child #2. During the time that this chaos was occurring, 2 additional staff members were attempting to lead the remaining 14 children in a group activity in the far southeast corner of the room away from the 2 children who were displaying these aggressive behaviors. The Surveyors could see that the children in the group were carefully watching the 2 children, and some appeared to be fearful of their actions. The Surveyors interviewed staff regarding the 2 children and their behavioral issues. Staff stated the following: * This behavior happens every single day. * Child #1 curses consistently and yells curse words at staff and other children. * Child #1 throws toys across the room and destroys other children's building structures during center time. * Child #1 likes to slap and has punched staff in the stomach, bit staff on the finger and hit staff. * Child #1 has kicked staff in the chin a few times. * Child #1 bit me on the arm today. * I have been hurt physically and mentally by child #1. * Child #2 screams, hits, bites, and pulls hair. * Both child #1 and child #2 have hit other kids in the room. * Both child #1 and child #2 climb on shelves and tables, and throw toys. The other kids have been hit by flying toys and have been hurt. * A child got kicked in the eye by child #1 because the child did not move away from child #1 when child #1 was kicking their feet. * We have asked for help, but even when someone comes in, the children are difficult to control. * These behaviors are not happening every now and then, it is every single day! * Child #1 has screamed in my face and called me curse words. * Child #1 yells curse words at the staff and the other kids in the room. * I have seen other kids get very nervous and begin to shake when child #1 is acting out. * Child #1 hit me in the back and hit me in the head. * Child #2 bit a teacher on the leg and spit on a teacher. * Child #1 throws blocks and toys. Other kids have been injured from flying toys. They have been bruised and cut. * One child even sustained a black eye from child #1. * Another child has been hit by toys that have been thrown across the room. * Both child #1 and child #2 climb on tables, throw food, and swear at the teachers and other kids. * We almost need 5-8 staff just to watch child #1. * Child #1 jumps up on tables and tears up other children's projects. * Child #2 throws things across the room and has hit other kids with the objects. * Child #2 pulls other children's hair. Nine accident/injury reports were provided for the Surveyors' review. Per these reports, enrolled children were injured from a direct result of aggressive behavior on the following dates: 09/24/19, 11/14/19, 11/15/19, 01/04/2020, 01/09/2020, 01/23/2020, 01/27/2020, 01/29/2020, and a second injury on 01/29/2020.

Findings: Based on facility documentation and the Surveyor's observations, it was determined that the prescription number on the medication box did not match the prescription number documented on the written authorization form for child #1's medication.
