Findings: Per Allegation and based on staff interview and the Surveyors' observations, in the Two's Classroom, the health, safety, and welfare of enrolled children was placed at risk of harm on 7/6/2021 when two-year-old children were observed to be unclothed in the outdoor activity area, and were able to be easily seen by individuals in a nearby apartment building and school/church parking lot. Two's classroom staff stated that as needed following water play, they would have the children undress then would spray the sand off of the children with a hose. Once done, the children would get their towels and dress inside the classroom.
