Findings: Based on the Surveyor's observation, the following items were stored in the restrooms and not maintained in a clean condition as follows: *Room 101 - a table, 2 gates, child-size shopping cart. *Room 105 - 4 cots and 2 storage shelves contained children's extra clothes and diapers which were spilling out onto the floor. *Room 205 - a large storage shelf contained 32 small, 5 medium, and 2 large containers labeled art materials. An empty mop bucket and magnetic gear toy were located on the floor. *Room 207 - an art drying rack and child-size chair. A sheet of paper was stuck to the wall behind the toilet. *Room 209 - an art easel was located near the door and 5 books were located on the counter of the hand-washing sink.
