| Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of three years for the date of issuance). It was determined based on the consultant's observation of the staff and training worksheet that a staff member hired on February 16, 2019, had a background check that expired on February 12, 2022. | |||
