Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. A safe indoor and outdoor environment was not provided for the children. 'There was a tub containing 2-3 inches of water and a broken pink kiddie pool on the landing of the steps in the outdoor learning environment. This was corrected when the director poured out the water and emptied the tub. She also removed and disposed of the pink kiddie pool.'
