| A safe indoor and outdoor environment was not provided for the children. On the playground there was exposed concrete around the basketball pole and the basketball goal had a rusty area. The stakes around the border of the playground are sticking up higher than the border. There were rusted screws, a loose screw, and screws that were not flush to the board at the music garden. Roots were exposed on the playground used by toddlers. There was not enough mulch in the fall zones of two sliding boards but teachers pulled mulch forward while I was standing there. | |||
