The Plan of Corrections is requested to be submitted in the LMS portal within 10 days of receipt. A copy of the Notice of Inspection Rights was provided at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. 2 of 2 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. | |||

| Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 04.9.25 and are subject to changes pending programmatic review. The Written Plan of Corrections is required to be submitted in the Licensing portal within 10 days of the receipt of the Statement of Deficiencies. 2 of 2 Fingerprint cards reviewed were valid via DPS website search. The following was discussed but not limited to: 1) The staff's date of employment, as it relates to staff’s obtaining a fingerprint clearance card, the licensee verifying the validity of the staff’s fingerprint clearance card, staff's completion of orientation training, staff's annual training cycle, staff's completion of TB test etc, for both new employees and re-hired employees, 2) The DCS portal that was effective in August 2024 and that is now required to be used, and 3) The requirements regarding children who have religious or medical exemption requirements that the child will not be able to attend during a communicable disease outbreak. | |||
