Initial Comments: The purpose of the inspection was to conduct a Complaint investigation 133594 on 6.25.2025. This was a self-report. A full inspection was not conducted at this time. Please submit the Plan of Corrections via the LMS portal within 10 days. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Designee at the time of the inspection. Ratios observed were as follows: School age: 1:11, 1:12, & 1:14 There were 3 staff interviewed during this investigation. There was 1 child interviewed during this investigation. There was 1 staff file reviewed during this investigation. 1 of 1 Fingerprint Clearance card reviewed was valid via the DPS website search. The following documents were observed: written documentation related to the allegations. Others interviewed: The complainant. Upon completion of the Complaint investigation 133594 it was determined from the Compliance Officer's observations of documentation and interviews that 2 of 2 allegations were substantiated. The following deficiencies were observed and are subject to changes pending programmatic review. The following was discussed but not limited to: 1. Mandated reporting.
INSP-0132959 2025-06-04 Compliance (Annual) Complete
Initial Comments: The following deficiencies were observed at the time of the annual compliance inspection conducted on 6/4/2025, and are subject to changes pending programmatic review. A full inspection was conducted at this time. 5 of 5 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Please submit the Plan of Corrections via the LMS Portal within 10 days of the receipt of the Statement of Deficiencies. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The following was discussed, but not limited to: 1. Playground maintenance. 2. Facility maintenance. 3. Staff file documentation. 4. Children’s sign in and sign out sheets.

Initial Comments: The following deficiency was found at the time of the Compliance Inspection conducted on 6/11/2024, and are subject to changes pending programmatic review. Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. Please submit a copy of the current fire inspection when obtained, since Fire inspection has recently expired. There were 4 staff files reviewed. 4 of the 4 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Assessment was completed at the time of the inspection.

Initial Comments: The following deficiency was found at the time of the compliance inspection conducted on 6/13/2023 and is subject to changes pending programmatic review. Name of Compliance Officer: Jennifer Flicker The following was discussed: The 2 types of child immunization exemptions that are accepted by the Department- Religious and Medical Exemptions. The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. There were 4 staff files reviewed. Four of the 4 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Program Assessment was also completed at the time of the inspection.
