Findings: The file for staff #2 stated that he was employed as a teacher and the regional manager stated that the staff was hired to work as a teacher. The file for staff #2 contained documentation of the staff's transcripts verifying that the staff had at least 12 credit hours from an accredited college or university, including at least six credit hours in early childhood, child development, or a closely-related field. The file reflected that the staff had only 3 months of childcare experience at the time of hire and did not meet the minimum qualifications of a teacher caregiver at the time of hire.

Findings: During an inspection of the facility, the Surveyor observed the following: *Outdoor activity area-Fourteen empty drinking cups were observed on the ground throughout the playground; *A 15 foot section of the hallway wall outside of room #108 was covered with dirt and brown smudges; and *Numerous pieces of toilet paper and paper towels were observed on the floor in both bathrooms that are located near the Multi purpose room.
