Initial Comments: The following deficiencies were observed at the time of the initial monitoring inspection conducted on 9/11/2025, and are subject to changes pending programmatic review. Please submit the Plan of Corrections through the Licensing Portal within 10 days of receipt of the Statement of Deficiencies. Three of three Fingerprint Clearance Cards reviewed were valid via the DPS website. The following was discussed, but not limited to: *New rules effective 8/03/2025. *Volunteer and visitor requirements. *Food waste is covered with a lid. *6 inches of resilient surfacing under fall zones.

Four of four Fingerprint Clearance Cards reviewed were verified as valid via the DPS website. The following was discussed, but not limited to: *Starting date of employment for all staff members. *Ensure the food menu is descriptive for all specific food types. *Clean the climber on the playground before the first day of school. *Address the gap under the south gate to the playground. *Ensure children in indoor and outdoor activity areas are supervised at all times. *Place outlet covers over the outlets in the Gym before use by enrolled children. *Clean all toilet rooms before the first day of school.
