Initial Comments: The purpose of the inspection was to conduct complaint #00162770 investigation on 3/27/2026. A full inspection was not conducted at this time. A copy of the Notice of Inspection Rights was provided at the time of the inspection. Ratios observed were: Infants - 2:7 1s and 2s - 1:6, 2:12 2s - 1:8 3s and 4s - 1:9 There were 4 staff members interviewed during this investigation. There were 2 children’s files reviewed during this investigation. The Compliance Officer contacted the complainant via phone on 3/20/2026. Documentation observed was: *Rosters *Incident reports *Staff attendance reports Upon completion of the complaint investigation, it was determined from observation, interview, and documentation that 1 of 2 allegations was substantiated. The other 1 of 2 allegations was unable to be substantiated due to a lack of sufficient evidence. The following deficiencies were observed and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The following was discussed, but not limited to: *Ensure the Accident, Injury, Emergency, Illness, and Infestation Log is being completed to document the applicable occurrences.

Initial Comments: The following deficiencies were observed at the Initial Monitoring Inspection conducted on 10/14/2025, and are subject to changes pending programmatic review. Please submit the Plan of corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. 5 of 5 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The following items were discussed, but not limited to: 1. Ensure the use of Individualized Plans for children who need extra support. 2. Ensure children's personal products are labeled with the child's first and last name. 3. Post the dated, weekly menu on the main posting board and ensure substitutions are documented. 4. Ensure facility equipment, such as rakes, is inaccessible to children on the playground. 5. Include all of the required statements in the facility's statement of services. 6. Ensure water is available to meet the needs of all enrolled children.

Initial Comments: There were no deficiencies observed at the time of the Initial inspection conducted on 8/20/2025. A full inspection was conducted at this time. 14 of 14 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The following items were discussed but not limited to: Ensure sufficient shade is available in the Infant's, Ones, and Two's playground. Infants must be in a wall-enclosed room.
