Noncompliance Area: A play set, which consisted of swings and a sliding board had been embedded in the back yard, which serves as the outdoor play space. The play set was not mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission.

Correction Required: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair.

Noncompliance Area: Observed in the first aid kit, antibiotic cream, antiseptic wipes and alcohol prep pads, that did not identify the name of the medication and the name of the child for whom the medication is intended.

Correction Required: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container.

Noncompliance Area: Observed one electrical outlet that lacked a protective receptacle cover on the surge strip under the desk in the living room.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: Observed bottom strip of siding at the entrance to the home and at the bottom of the kitchen door peeling. Observed broken, splintering wood on two sections of the fence in the outdoor play space

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play area, shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Observed peeling paint on the right wall between the stairway and the dining room.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: Observed several plastic shopping bags, which were accessible to children, in an unlocked closet in the Kidz Korner Room.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: The vehicle used for transportation did not include a first aid kit.

Correction Required: A first-aid kit, including the contents as specified in 3290.73 (relating to first-aid kit) shall be in the vehicle when children are being transported.

Noncompliance Area: The flat screen TV in the living room was not attached to the stand, which created a potential toppling hazard.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: Observed on the window ledge in the bathroom, can of air freshener and bottle of toilet disinfectant, which were accessible to children.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: A plastic bag with pom-poms and other art supplies less than one inch in diameter was observed in the living room. This was accessible to children who are still placing objects in their mouths, creating a potential choking hazard.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: The most recent Child Service Reports in the files for Child #1 through Child #6 exceeded six months between reports. 2/28/17, 2/2/18. The most recent Child Service Reports in the files for all six children are current.

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at 3290.124(e).

Noncompliance Area: The Emergency Contact forms in the files for Child #4, #5 and #6 did not include the addresses of the release persons.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: Steak knives were observed in the kitchen drawer, which was accessible to children. The drawer did not contain a childproof device.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: The most recent Parent updates for the Emergency Contact forms and Fee Agreements in the files for Child #1 through Child #6 exceeded six months between updates. 2/28/17, 2/2/18. The most recent updates in the files for all six children are current.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: Two outlets on the surge strip under the computer table did not contain protective receptacle covers. One outlet in the bathroom did not contain a protective receptacle cover.
