Initial Comments: The following deficiencies were observed at the time of the Annual Compliance Inspection conducted on October 15, 2025, and are subject to changes pending programmatic review. A full inspection was conducted at this time. Two of two fingerprint clearance cards were verified to be valid through the DPS website at the time of the inspection. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The Empower Self-Evaluation was emailed to the facility. The following was discussed, but not limited to: *Updated DCS Background Check process *Discussed updated rules and forms *Copy of current fingerprint card in staff file *Emergency drills and procedures Please complete the Plan of Corrections via the LMS Portal within 10 days of receipt of this Statement of Deficiencies.

Initial Comments: The following deficiencies were found at the time of the Annual Compliance Inspection conducted on October 24, 2024, subject to changes pending programmatic review. A full inspection was conducted at this time. Two of two fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Inspection Item Expiration Dates: Insurance: 04/01/25 Fire: 12/20/25 (State Fire Marshal) Gas: 02/05/25 Sanitation: 01/31/25 Items discussed, but not limited to: -State Fire Marshal inspections for are one year -Minimum of first initial & full last name on sign in/out -Monitor condition of wood equipment on playground -Updated DES/DCS Background Check process -Updated Entrance Letter -Second parent on Emergency Card (line through or N/A) -Forms and rule set available on AZDHS website The Plan of Correction was not accepted at the time of the inspection.
