| Initial Comments: The following deficiencies were observed at the time of the Initial Compliance Inspection conducted on 3/30/26 and are subject to changes pending programmatic review. A full inspection was conducted. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. The fingerprint clearance cards for 2 of 2 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: - Total capacity limits due to space - Household members files - Cots storage, sheets - Diaper-changing procedures and area - Outdoor yard: Supervision around corners of building, shade, temperature of turf - Driveway area (entrance)- maintenance, cleaning - Staff member files, content and maintenance, including training, fingerprint clearance card validation and tuberculosis documentation - Supervision - Main posting area documentation - Emergency Information and Immunization Record cards and attached Immunization records - Mandated reporting for suspected abuse or neglect - Drinking water availability - Hazards – maintenance, toxic/hazardous materials and equipment inaccessible - Food service - Discipline and guidance requirements - Cleaning (facility, toys & equipment) and functioning plumbing - Accident and Emergency Procedure regulations and documentation - Illness log maintenance and use, - Medications (permission forms, medication labeling, documentation, storage) - Fire and safety requirements (fire drills, fire extinguishers, outlets covered, no extension cords, cord accessibility, mounted and inaccessible fans) - Tummy time resources - Department access to home and licensed areas, inspections unannounced | |||
