|Findings: Upon arrival at the Home, in the playroom (near kitchen) a small child was observed sitting in a feeding chair which was stacked on top of two additional feeding chairs. *Previously cited 4/15/2020 (different scenario)|

|Findings: In the bathroom used by the enrolled children, there was a toilet bowl brush observed on the floor next to the toilet, accessible to the children.|
|Findings: In the bathroom used by the enrolled children, the roll of toielt paper was observed sitting on a metal shelf next to the toilet and not mounted, as required. The provider stated they just are finishing up some renovations and the toilet paper mount still needs to be installed.|
|Findings: The diaper changing surface (table) was observed to have the following on top of it: *binders *folders *small sewing machine|
|Findings: There was no soapy water bottle or bleach water bottle available for use to properly clean and then sanitize the diapering surface before and after each diaper change, as required.|
|Findings: Based on the Surveyors' observations, Surveyor #1 rang the doorbell three times over a 5 minute period as well as knocking on the door three times. Both Surveyors observed a child inside the house as well as children in the back yard. No one answered the door during this time. Surveyor #1 was able to enter the home as the door was unlocked.|
