All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. The front of the dishwasher and the cabinet next to it, had visual signs of something spilled on them. The floor near the classroom was sticky. In the office/dining room space, there was a crushed metal soda can with sharp edges on the child size table against the wall. The pack-n-play used for the infant in care, had one side that was collapsed and could not be fixed. In the bathroom used by the children, the toilet paper holder was broken, and the metal pieces were still attached to the wall and could injure a child using the toilet. The toys in the outdoor storage container were covered in mildew, dirt, and grass cuttings.

Indoor stairs with more than two steps were not made inaccessible to children two years of age or younger. There was no barrier blocking the stairs leading to the second floor when I arrived.

Electrical outlets not in use were not covered. In the spaces used by the children, there were broken outlet wall plates in the office space under the window and behind the couch. The plate coverings were missing from the outlet under the AC unit in the living room and the outlet next to the doorway to the children’s classroom. There was a wire protruding from the outlet next to the classroom with bare wire exposed.

There was a curling iron located in the cabinet under the bathroom sink. There was a heating pad, located on a pile of pillows, in the corner of the office/dining room.

Plastic bags, toys, and toy parts small enough to be swallowed, including but not limited to materials that can be easily torn apart such as foam rubber and styrofoam, were accessible to children under the age of three. In the living room, there was a plastic bag with a blanket inside and an empty plastic chips bag behind the couch where children were seen to go.
