Indoor and outdoor area was not checked daily for debris, vandalism, broken equipment, and animal waste. The paint on the handrails leading from the back porch to the playground were peeling and rough. There was a broken ceramic plant pot located beside the step to the storage shed at the back of the playground. A portable climbing structure had a crack in the exterior plastic and foam insulation was accessible to the children.

There was a crack in the plastic play house doorway with sharp edges that could injure a child as they go in or out. A black inner tube bouncy toy was hot to the touch. The new swings, on the swing set, were attached to the chain with “S” hooks which can catch children’s clothing causing a hazard.

A stack of Styrofoam cups in a plastic sleeve were located in the bottom of the dining room side board. This space is used by the children when they eat and do art activities. Today, there were three (3) children under the age of three(3) in attendance.
