Findings: A complaint investigation was conducted this date. Caller indicated that a 9 month old has a injury to his nose that looks like a burn. The provider said the infant scratched himself but did not provide an incident report. The provider stated that the child already had a small scratch on his face when he came to day care on Monday from the past weekend. Provider discussed with the mother that she needs to cut his nails because the child scratched her and the parent told the provider that he scratched her also. The parent then told the provider that she will make sure she cuts his nails. On Tuesday, after nap time the provider noticed that the child had a scratch on his nose. The provider inform the parent of the incident when she arrived to pick him up. The provider states she didn't write any incident report because the child scratched himself and she didn't think she needed to. Informed the provider that she should always document any type of incident that has occurred especially if you see a scratch or scar. | |||
