Initial Comments: The purpose of the inspection was to conduct a Complaint investigation #00151227 on 12.10.25. A full inspection was not conducted at this time. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the facility director at the time of the inspection. A plan of corrections will not be accepted through the portal at this time due to enforcement action. Ratios observed were as follows: Infant #2 2:5, 1:10 Toddler #1 (1y) 3:7 Toddler #2 1y) 3:6 Pre-K 2:15 Preschool 2:1 Discovery #1 (2y) 1:6 Discovery Preschool #1 (2y) 2:4 There were two staff members interviewed during this investigation. The following documents were observed: Incident reports, staff training attendance, and staff involuntary termination letter. The complaint was self-reported. Upon completion of Complaint #00151227, it was determined from the Compliance Officer's observations, facility documentation, and staff interviews that two of two allegations were substantiated. The following was discussed, but not limited to: *Ensure cots are separated 18 inches during nap time. *Ensure lights are dimmed during nap time.

Initial Comments: The following deficiencies were observed at the time of the initial monitoring inspection conducted on 09.29.25, subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Bill of Rights to the facility director at the time of the inspection. The following was discussed, but not limited to: *Please provide the entrance code to the department.
INSP-0137793 2025-07-30 Compliance (Initial) Complete
Initial Comments: The following deficiency was observed at the time of the Initial inspection conducted on July 30, 2025. A full inspection was conducted. Please submit the plan of correction, along with documentation of the correction, through the licensing portal prior to approval of licensure. Twenty-two staff files were reviewed, the fingerprint clearance cards for all staff files were verified as valid via the DPS website. The following was discussed, but not limited to: *Complete staff files prior to starting employment, *Ensure staff members' positions reflect their qualifications, *Ensure toilets are maintained in an operable condition, *Ensure diaper changing water is maintained between 86°F and 110°F, *Ensure sanitizing solution for the diaper changing area is approved by the Department.
