| Initial Comments: No deficiencies were found at the time of the Initial Inspection conducted on 06.22.2026. This report is subject to changes pending programmatic review. A copy of the Notice of Inspection Rights was provided at the time of the inspection. A Plan of Corrections is not required at this time. There were 2 staff files reviewed. 2 of the 2 fingerprint clearance cards were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but not limited to: 1. The outdoor tree stump must be removed, 2. Exposed concrete and holes must be covered, 3. Tricycle handlebars must be replaced, 4. Drinking fountains must be clean, 5. All outlets must be covered, 6. Strings and cords must be inaccessible, 7. Menu items must be specific, 8. Background checks must be received before the employee's start date, 9. Children's Emergency, Information, and Immunization Record cards must be attached to the immunization records, 10. Suspected child abuse must be reported, 11. Staff must have 24 hours of training annually, 12. Fire drills must be completed monthly, 13. Children with special needs must have an individualized plan, and 14. The Department must be notified of any closures. | |||
