Violation Observed: Children were observed using two wheel bikes without helmets. Ensure children wear helmets if they use the bikes and that the helmets are washed and disinfected between uses.

On the day of the inspection a child was observed playing with and sharing her stuffed animal while out on the playground. The center does not have means to launder the stuffed animal between uses.

Violation Observed: On the day of the inspection there was only one staff person with 2 different groups of children outside and she was sitting on a chair not supervising any of the children. Ensure that staff are actively and appropriately supervising children at all times.

Violation Observed: On the day of the inspection there was one staff person on the playground with 12 children when two staff were needed to maintain adequate ratios. Ensure ratios are maintained at all times.

Violation Observed: On the day of the inspection when staff were questioned about the number of children in their care and their ages they were unable to answer the question. Ensure staff are fully aware of the number of children in their care and the ages of the children at all times.

The center shall be permitted to obtain a valid fire safety inspection certificate issued by the municipality in which it is located, based on a fire inspection conducted within the preceding 12 months, and submit a copy of the certificate to the Office of Licensing in lieu of a CO or CCO, if the center serves only children 2 ½ years of age or older and is located in a public school building that is used as a public school.

Swimming pool requirements are as follows: wading pools that are 24 inches or less, are filled as needed, are portable, and do not have a filtration system are prohibited.

Violation Observed: On the day of the inspection, observation indicated and interviews revealed that the staff fill the pools with water so that the children and enter into the and have water play. Wading pools are not permissible. Cease the use of wading pools.

Space and room requirements are as follows: all space and rooms within the center to be used by children shall be inspected and approved by the Office of Licensing prior to their use. In making its determination, the Office of Licensing shall consider whether the space is too far removed, remote, or isolated from other areas of the center to be used by children. rooms or areas of the center that are not Office of Licensing-approved for use by children shall be made inaccessible to children.

Violation Observed: On the day of the inspection interviews with the staff indicate that during the year there is a prek classroom conducted on the first floor, and on the day of the inspection a staff person (whom does not work for the center in the summer, as per interview) was observed providing specialized instruction to an enrolled child. Ensure all areas that are used by the children are formally approved by OOL.
