Center must ensure that ratios are maintained during this time of the day. Violation abated during inspection as director responded immediately to the issue.

Violation Observed: Ensure that staff wash their hands with soap and running water. On the day of the inspection a staff person in the infant did not remove her dirty glove used during diapering into the dirty diaper, and it remained on as the child was brought back into the environment.

Violation Observed: Ensure that adequate supervision is maintained at all times throughout the center. On the day of the inspection several staff were observed on their cell phones when they were watching the children and/or putting the children to sleep. Ensure staff remain off their cell phones while working with children or excuse themselves and arrange for coverage when they need to use their phones.

At least five foot-candles of natural or artificial light shall be provided in all rooms used by children during naptime. The illumination shall be measured three feet above the floor at the farthest point from the light source.

Violation Observed: Provide an adequate amount of light in the classroom during naptime. Room in the basement did not have enough light in the room to see the children while they were napping. The room was observed again and the lighting had been reduced again. Ensure there is enough light in the room for staff to be able to see the children at all times.

Violation Observed: Ensure the health, safety, and well-being of the children served. on the day of the inspection observation indicated that an infant was placed to sleep in a pack and play that was labeled with the correct child's name along with their bedding, but a different child was placed in the pack and play to sleep. Ensure children are correctly placed in their identified sleeping equipment.

For early childhood programs, the center shall provide opportunities for daily rest and sleep as follows: swaddling of children during rest and sleep is prohibited.

Violation Observed: Cease swaddling infants. On the day of the inspection an infant was observed in a swaddler sleep sack. Staff were not aware that infant are never to be swaddled while in care. Infant was removed from the sleep sack at the time of the inspection, but staff must be retrained to ensure that no other infant is swaddled again and parents must be corrected in sending swaddling sleep apparel to the center. Submit documentation which demonstrates that staff and parents are aware that swaddling is not allowed at the center.

For early childhood programs, the following shall apply: diapering requirements for centers serving children who are not toilet trained are as follows, soiled diapers shall be disposed of as follows, soiled disposable diapers shall be placed in a closed container that is lined with a leakproof or impervious lining. Such diapers shall be removed from the center daily and placed in a closed garbage receptacle outside the building.

Violation Observed: Ensure that soiled disposable diapers are placed in a closed container that has a leak proof lining. Dirty diapers observed in a garbage can with no cover.

The director or his or her designee(s) shall be on the premises at all times when the center is operating and the designee shall not have full-time classroom responsibilities during the times when the director is not present at the center.

Violation Observed: Designate a director designee for whenever the director is not present at the center and ensure that the director designee does not have full-time classroom responsibilities during the times the director is not at the center. On the day of the inspection the director designee had no idea how to contact the central monitoring center when the center's fire panel was silenced and in trouble. Ensure director designees are completely trained to handle situations in the absence of the director and the sponsor.
