Description: Health Information/ACIP recommended immunization record

Noncompliance Area: Documentation showed that the earliest health assessment for child #2 was dated 8/31/25, exceeding the requirement that initial health assessments be obtained within 60 days of enrollment. The earliest immunization report was dated 8/28/25, exceeding the requirement that initial immunization reports be obtained within 60 days of enrollment. See LIS for child's date of enrollment.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP.

Correction Required: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler.

Description: Exemption documentation from parent/guardian/Exemption documentation from parent/guardian

Noncompliance Area: Documentation showed that child #1 had not yet received Varicella of Hepatitis A immunizations - See LIS for child's date of birth. Reportedly, the child is on a delayed schedule, however, a letter of exemption was not available.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP. Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. The statement shall be kept in the child's record.

Description: FIRE DETECTION/Staff responsibility

Noncompliance Area: Documentation showed that a fire system test was conducted on 1/23/26 and again on 2/24/26 (32 days), exceeding the requirement that test be conducted at least every 30 days. Documentation showed that a fire system test was conducted on 3/26/26 and again on 4/26/26 (31 days), exceeding the requirement that test be conducted at least every 30 days. Corrected on site by testing the system during the inspection.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. §  1016(c)). The primary staff person or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met

Regulation: 3280.105(a)

Description: Clean, age appropriate

Noncompliance Area: Children's sleeping rolls were not labeled with their names and 2 pack-n-plays were not labeled for individual child use.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Description: Written emergency plan posted

Noncompliance Area: The Emergency transportation plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency, was not posted in the Dining Room, Front Room or Back Room childcare spaces.

Correction Required: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.

Description: ACIP recommended immunization record /Dismissal policy

Noncompliance Area: The health report and immunizations for Child #1 did not include documentation of the HEP A vaccine having been administered in accordance with the ACIP recommended schedule. There was no written, signed medical or parental exemption statement for the HEP A vaccine in the file. Child #1 was still attending without this documentation, beyond 60 days of the child's first day of attendance at the facility.

Correction Required: The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Code § 27.77 (relating to immunization requirements for children in child care group settings).

Noncompliance Area: Bottles used for Child #2 were not labeled with the child's name.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Description: Locked or inaccessible - in original container

Noncompliance Area: A spray bottle containing bleach and water, was on the island counter, accessible to children who were sitting at the island, coloring (corrected on-site).

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: The backpack containing a first-aid kit, was located on a chair in the garage, accessible to children who were playing outside on the driveway (corrected on-site).

Correction Required: A first-aid kit shall be inaccessible to children.
