Description: Small Toys and Objects

Noncompliance Area: Three packages of wiped labeled with warnings to keep bags out of reach of children were stored on the bottom shelf of the changing table, accessible to children. Corrected on site by moving the wipes to a high shelf.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: Documentation of a 6-month health assessment was not available for child #3, who was 1 year and 5 months old at the time of enrollment. Documentation of a 6-month health assessment was not available for child #4, who was 1 year and 5 months old at the time of enrollment.

Correction Required: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler. A child's record must contain initial and subsequent health reports.

Description: Written instructions/Written consent

Noncompliance Area: Documentation of written instructions and parent consent to administer medication was not available for child #1. Documentation of written instructions for the administration of medication was not available for child #2.

Correction Required: A staff person shall administer a prescription medication only if written instructions are provided from the individual who prescribed the medicine. Instructions for administration contained on a prescription label are acceptable. A parent shall provide written consent for administration of medication or a special diet.

Description: Mantoux TB

Noncompliance Area: Documentation of the date that the TB test was read was not available on the health assessment for staff #2.

Correction Required: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Noncompliance Area: Sunscreen was observed in 3 children's bags that were on low shelves/cubbies, that were accessible to children (corrected on-site).

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: The First-aid kit in the classroom was on a low shelf, accessible to children and the first-aid kit in the backpack that is taken outside with the children to the play yard, was hanging on a low hook, also accessible to children during the inspection (both corrected on-site).

Correction Required: A first-aid kit shall be inaccessible to children.
