Noncompliance Area: The sharp edges of 6 screws were protruding from the door of a child sized cabinet causing sharp edges to be accessible to the children. The trampoline in the backyard play area, which is reportedly not used by the children in care, had a ripped lining that hung down in a loop causing a strangulation hazard to be accessible to children.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: The fire extinguisher in the kitchen of the facility had an inspection tag attached to it that documented that it had not been inspected since August 2017.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: Staff person #1, staff person #2 and Staff person #3 had NSOR verifications that were not obtained until 7-9-20, which was after the required time frame as designated by the CPSL.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: There were uncovered outlets on a power strip on the floor in the living room accessible to children. There was an uncovered outlet on the wall in the bathroom accessible to children.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: There was a container of laundry detergent labeled Keep Out of Reach of Children accessible to children on the floor in the basement childcare room.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: The bathroom window on the second floor of the facility was open more than 6 inches and was accessible to the children.

Correction Required: Windows or doors above the ground floor that open directly to the outdoors and are accessible to children shall be constructed, modified or adapted to limit the opening to 6 or fewer inches.

Noncompliance Area: 2ND CITATION: Child #1 and Child #2 had immunization records that lacked the influenza vaccination as recommended by the ACIP. PREVIOUSLY CITED ON 6-11-19

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play area, shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: The rest equipment in the upstairs bedroom was not labeled with the intended user's names.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: Child #1 had an agreement that lacked the child's date of admission.

Correction Required: An agreement shall specify the date of the child's admission.

Noncompliance Area: Child #3 had an emergency contact form that lacked the child's health insurance policy number.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: Child #1 had a most recent health assessment dated 11-20-17.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.
