The sponsor or sponsor representative, and each staff member shall complete a signed consent form provided by the Department that indicates the identifying information necessary to conduct a CARI background check, including the person’s name, address, date of birth, sex, race, and Social Security number. Pursuant to the Federal Privacy Act of 1974 (P.L. 93-579), the Department shall advise each such person that the disclosure of his or her Social Security number is voluntary, and that the Social Security number will only be used for the purpose of conducting a CARI background check.

Violation Observed: Ensure that CARI background checks are completed as required for all staff within two weeks of hire. There are 7 missing CARI checks.

As a condition of securing a license or Certificate of Life/Safety Approval, the sponsor or sponsor representative shall ensure that a Criminal History Record Information (CHRI) fingerprint background check is completed for himself or herself, and for all staff members at least 18 years of age who are or will be working at the center on a regularly- scheduled basis, to determine whether any such person has been convicted of a crime, as specified in P.L. 2000, c. 77 (N.J.S.A. 30:5B-6.10 to 6.17). The sponsor or sponsor representative and each staff member shall complete the electronic fingerprinting process through the vendor authorized by the State to conduct CHRI background checks through the Division of State Police in the Department of Law and Public Safety and the Federal Bureau of Investigation.

Violation Observed: Ensure that all children are strapped into infant equipment as per manufacturers instructions. There was a child in room 1 that was sitting up in a bouncer seat and not strapped in.

Violation Observed: The center must provide a MOU to the Office of Licensing stating as per ASTM standards, only the correct age group, children ages 2-5 years old, will be utilizing the large blue and white play equipment in the outdoor play area. If in the future, the center obtains or changes any play equipment in the outdoor play area the center will ensure that all ASTM standards are met and only the proper ASTM certified age groups utilize the equipment. The center will also maintain manufacturers documentation indicating all play equipment meets ASTM standards on file at the center.

The children shall be supervised directly by a staff member at all times, including during outdoor activities, rest and sleep, and walking through hallways. Toileting procedures shall be supervised by a staff member as appropriate for the ages and developmental needs of the children.

Violation Observed: Ensure that adequate supervision is maintained at all times throughout the center. Infants were sleeping in cribs located behind a tall cabinet, cribs were lined up in front of each other, and all classroom staff were on located on the other side of the room. Staff in room 3/4, were not able to provide direct supervision to children sleeping due to a lack of visibility.

For early childhood programs, the center shall provide opportunities for daily rest and sleep as follows: swaddling of children during rest and sleep is prohibited.

Violation Observed: Cease the use of swaddlers for infants and ensure sleep sacks are used properly. An infant in room 1 had on a swaddler that was too small and the child's legs were wrapped in the material of the sleep sack.

Violation Observed: Cease the use of the large climbing equipment in the outdoor play space or provide documentation showing the age group for which the equipment is to be used. The center will need to provide an MOU stating they will no longer have children using the equipment and it will be blocked off. At the time of inspection children ages 2.5 to 3 were using the large climbing equipment. 4-7-22: Provide ASTM documentation for the new playground equipment.11-4-22: The center removed all climbing equipment from the outdoor play space. See center MOU.

Violation Observed: Ensure the gate in the back right corner of the outdoor play space is closed at all times. At the time of inspection the gate was open and children were on the playground.

Violation Observed: Ensure that children are exiting out of the back doors of the building to the playground, not the front door and walking around the side parking lot. The center is located on a very busy road and cars can pull head on into where the children are walking.

Room 5 had 16 children present and the room's capacity is 15.

Violation Observed: Ensure sleeping equipment is used according to how it is manufactured. A swaddler had part of the material cut off which resulted into holes in the material large enough for the infant to get tangled in it.

Great staff- take care of children very well! Kids come home happy, clean, well tended to. Definitely recommend.
