The sponsor or sponsor representative and each staff member shall complete the electronic fingerprinting process through the vendor authorized by the State to conduct CHRI background checks through the Division of State Police in the Department of Law and Public Safety and the Federal Bureau of Investigation.

Violation Observed: The center must provide a new playground certification letter to the Office of Licensing which states the correct age groups for the playground equipment located in the outdoor play area. The center provided OOL an altered certification letter changing the permitted age group of children permitted to use the equipment. The equipment is certified for children ages 5-12 years old however the center provided a letter stating the equipment was for 2-12 year old use. 10-3-2022- The center removed this playground equipment.

Indoor maintenance and sanitation requirements are as follows: all corrosive agents, insecticides, bleaches, detergents, polishes, any products under pressure in an aerosol spray can, and any toxic substance shall be stored in a locked cabinet or in an enclosure located in an area not accessible to the children.

Violation Observed: Ensure that all toxic substances are inaccessible to the children. The 2nd floor staff bathroom was open and children had access to bleach and cleaners being stored in this bathroom.

Violation Observed: Ensure that all staff who do not have a completed CARI and CHRI background check are directly supervised by another staff member. A therapist who did not have a CARI or CHRI clearance was walking children through the hallways back and forth to classrooms.

An additional covering shall be provided for use as a covering for each child.
