Breast milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. The bottles that were prepared did not have the date the bottles were prepared. This was corrected by the teacher adding the dates to the bottles.

Screen time was offered to children under three years of age. In space #2, there were five children two years of age watching videos on tv screen, while dancing to music and movement songs. This was corrected by the teacher covering up the tv screen while the children listen and danced to the songs.

Center staff did not comply with the safe sleep policy. In space #4, an infant was asleep with in a crib with a pillow. In another crib were a child was laying, there was a blanket in the crib with the child. This was corrected when the teacher removed all items out of the cribs.

The center's safe sleep policy was not posted in a prominent place in the infant room where parents and caregivers were able to view daily. In space #4, The safe sleep policy was not posted. This was corrected by the operator posting the safe sleep policy on the parent board in space #4. This was corrected when the teacher removed the items from the crib.

The operator did not notify the parent of each child enrolled in writing of the smoking and tobacco restriction. Child (C.M.) did not have anything on file stating that the parent was informed about smoking and tobacco restriction. Parent signed the smoking and tobacco restriction policy during the visit today.

A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. Child (C. M.) did not have signed receipt from parent for Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy. It was corrected by having the parent to sign documents during visit. The parent is a teacher.
