All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. I space #3, there were was and air freshener plug in one of the outlets. The teacher removed it and place in a locked closet.

First aid kit and/or fire extinguisher was not located in vehicle; kit and/or extinguisher was not mounted or secured if in passenger area. The first aid kit and fire extinguisher was not in vehicle that was being used to transport children. The van driver went to where the the other van was being worked on and got both first aid kit and fire extinguisher and place them in the vehicle that was being used to transport children.

Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. The emergency information and photographs for each child that were being transported daily was not in the vehicle.

For routine transport of children to and from the center, staff did not use the list to document attendance as children boarded and departed the vehicle. Children that are routinely transported are not documented on an attendance documented of broadening and departing the vehicle.

Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. In space #3, the staff had a 16oz bottle of Pepsi sitting on a shelf. The staff member put the bottle of Pepsi in a closet.
