Regulation: 3270.102(f)/3270.104(a) Description: Indoor climbing- protective surface/Clean, good repair, proper size Noncompliance Area: On 4/30/26, at time of initial inspection, a 4'4" climbing structure including a slide was observed in the front corner of room MS4. The area beneath the unsecured 4'4" platform and slide was not equipped with a protective surface covering extending throughout the fall zones of the structure. Additionally, based on the manufacturer specifications, the slide is not designed for commercial use and has been modified from its intended installation (bottom of slide not embedded), therefore compromising its safety. Correction Required: Indoor play equipment for climbing shall be installed or used over a protective surface covering which does not interfere with the stability of the equipment. Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs. |

Provider Response: (Contact the State Licensing Office for more information.) 1. No Children will be permitted on the structure in MS4 (slide and platform) at any time - there will be signage hung. 2. There will be a large divider placed around the structure with signage hung on it, divider will be secured with zip ties so it cannot be moved. 3. All staff will be trained on the room MS4 before their first day of work at camp (6/15/26). Staff will be required to walk through MS4 with Director, understand the perimeter, read the training sheet on the structure and sign off that no children will be allowed on the structure or behind the blue divider at any time. *Sign off sheet will be placed in the staff file on site.
