Noncompliance Area: On 8/19/26, a tall, narrow bookshelf (approx. 6ft high) observed in School Age B classroom was not anchored or secured to prevent tipping. The bookshelf was unstable and could be pulled forward, creating a potential tip hazard to children.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Description: Staff assigned to specific children/Child's hands washed

Noncompliance Area: On 7/30/26, during unannounced inspection, Staff #1 sent a preschool age child to the bathroom located in Staff #2 classroom which is adjacent to their classroom. Upon finishing in the bathroom, certification rep observed child leave the bathroom without washing their hands. Although Staff #1 and Staff #2 were both aware of child #1 in the bathroom, neither Staff #1 nor Staff #2 were directly supervising the child to ensure handwashing. Additionally, Staff #3 was observed changing diapers in the young toddler room. Staff #3 did not wash child's hands after being diapered.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered.

Description: No physical punishment

Noncompliance Area: On 4/30/26, at approximately 4:55pm, Child #1 was on the floor in the doorway between the PS and PK2 classrooms exhibiting a tantrum. Staff #1 acknowledged holding Child #1 ankles and dragging Child #1 into the PK2 classroom.

Correction Required: A facility person may not use any form of physical punishment, including spanking a child.

Noncompliance Area: On 12/23/25, at time of inspection, a bowl of marbles with a diameter of less than an inch was observed accessible to toddlers on the sink of the classroom bathroom.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Child #1 had just woken up from rest time where it appeared the plastic edge of the rest cot likely left an impression on Child #1 forehead. Staff #1 did not complete and provide an injury report to the parent on the day of the incident.

Correction Required: A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file.

Noncompliance Area: On 1/10/25, a plant known to be poisonous (Wondering Jew) was observed in the PK1 classroom. Additionally, various plants known to be poisonous were observed accessible to children on the outdoor play space.

Correction Required: Toxic plants are not permitted in a child care space. Teachers will label all plants in room. Outdoor plants identified as poisonous/toxic will be removed by landscaping.

Noncompliance Area: On 1/25/24, at time of inspection, infant Child #1 and infant child #2 were observed sleeping in cribs located in the back designated sleeping area unsupervised by a staff person. Child #1 crib was behind a half wall and Child #2 crib was positioned behind a diaper changing station preventing supervision.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times.

Noncompliance Area: During unannounced monitoring certification rep spoke with staff and verified that the child's diaper was soak and that parent did not receive any report showing when the child was changed.

Correction Required: The operator shall establish oral or written communication in the language or mode of communication which is understandable to the parent. A staff person shall check a child's diaper at least every 2 hours and whenever the child indicates discomfort or exhibits behavior that suggests a soiled diaper. A staff person shall change a child's diaper when the diaper is soiled.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: ON 12/21/17, A KITCHEN PLAY SET OBSERVED ON THE OUTDOOR PORCH AREA WAS IN POOR REPAIR. A TORN FLOOR PLAY MAT WAS OBSERVED IN THE YOUNG TODDLER ROOM. A LARGE CONTAINER/BUCKET OBSERVED ON THE OUTDOOR PORCH AREA ACCESSIBLE TO CHILDREN WAS FILLED HALF WAY WITH RAIN WATER.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. A water play table or a container used for water play that contains unfiltered water shall be emptied daily.

Noncompliance Area: ON 12/21/17, STAFF #2 ACKNOWLEDGED THAT THE REFRIGERATOR LOCATED IN THE DIRECTOR OFFICE IS USED FOR REFRIGERATED MEDICATION WHEN NEEDED. A THERMOMETER WAS NOT OBSERVED IN THE REFRIGERATOR LOCATED IN THE DIRECTOR'S OFFICE.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45 F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: ON 12/21/17, TWO INFANTS WERE OBSERVED ASLEEP IN CRIBS LOCATED IN THE SEPERATE SLEEPING AREA ON THE FAR BACK WALL OF THE INFANT ROOM. STAFF #3 AND STAFF #4 WERE POSITIONED IN THE CHILD CARE SPACE WITH AWAKE CHILDREN AND UNABLE TO SUPERVISE THE TWO INFANTS WHO WERE SLEEPING IN THE CRIB AREA.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Children must be supervised at all times. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. Noncompliance Area: ON 12/21/17, THE HEALTH ASSESSMENT OBSERVED IN THE FILE BELONGING TO STAFF #7 (DATED 12/13/17) DID NOT INCLUDE THE PHYSICIAN'S ASSESSMENT OF SUITBILITY TO PROVIDE CHILD CARE.

Correction Required: An adult health assessment must include the physician's or CRNP's assessment of the person's suitability to provide child care.

Noncompliance Area: ON 12/21/17, NO EVIDENCE OF ANNUAL FIRE SAFETY TRAINING WAS OBSERVED IN THE FILE BELONGING TO STAFF #9 (HIRED 4/18/16).

Correction Required: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Noncompliance Area: ON 12/21/17, NO EVIDENCE OF AN APPROVED MANDATED REPORTER TRAINING WAS OBSERVED IN THE FILE BELONGING TO STAFF #1. THE MANDATED REPORTER TRAINING IN THE FILE WAS NOT CONDUCTED BY AN APPROVED TRAINING ORGANIZATION.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).

Noncompliance Area: ON 12/21/17, STAFF #5 AND STAFF #6 AND STAFF #8 ARE BEING UTILIZED AS ASSISTANT GROUP SUPERVISORS. NO EVIDENCE OF OFFICIAL VERIFICATION OF EDUCATION WAS OBSERVED IN THE FILE BELONGING TO STAFF #5 AND STAFF #8. NO EVIDENCE OF VERIFICATION OF 2 YEARS EXPERIENCE WITH CHILDREN WAS OBSERVED IN THE FILE BELONGING TO STAFF #6.

Correction Required: An assistant group supervisor shall have a high school diploma or a general educational development certificate and 2 years experience with children.A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity.

Noncompliance Area: ON 12/21/17, STAFF #2 ACKNOWLEDGED THAT YOUNG TODDLERS UTILIZE THE OUTDOOR PLAY SPACE WHERE CLIMBING EQUIPMENT IS LOCATED. THE CLIMBING EQUIPMENT LOCATED ON THE PLAY SPACE IS NOT SAFE AND DEVELOPMENTALLY APPROPRIATE FOR YOUNG TODDLERS. THE MANUFACTURER SPECIFICATION STATES THE PLAY EQUIPMENT IS TO BE USED FOR CHILDREN AGES 2 -12 YEARS. ON 10/26/17 DURNIG AN UNANNOUNCED VISIT, YOUNG TODDLERS WERE OBSERVED ON THE PLAY SPACE CONTAINING EQUIPMENT THAT IS NOT SAFE OR APPROPRIATE FOR THEIR AGE AND DEVELOPMENT.

Correction Required: Outdoor or indoor play space shall be safe for large muscle activity. Play equipment and materials appropriate to the developmental needs, individual interests and ages of the children shall be provided in sufficient amount and variety to preclude long waits for use.

Noncompliance Area: ON 12/21/17, 2 UNCOVERED ELECTRICAL OUTLETS WERE OBSERVED IN THE POWER STRIP ACCESSIBLE TO CHILDREN IN THE PK COUNTS CLASSROOM.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: ON 12/21/17, PEELING PAINT WAS OBSERVED ON THE WINDOW SILL ACCESSIBLE TO CHILDREN LOCATED IN THE TODDLER 2 CLASSROOM AND ON A WALL LOCATED IN THE PK 1 CLASSROOM. A HOLE WAS OBSERVED ON THE WOODEN HALF DOOR LOCATED IN THE ENRICHMENT ROOM. THE WALLS LOCATED IN THE ENRICHMENT ROOM WERE DIRTY. A STAINED CARPET WAS OBSERVED IN THE TODDLER 1 CLASSROOM.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: ON 12/21/17, A LID WAS NOT OBSERVED ON THE WASTE RECEPTACLE LOCATED AT THE SINK IN THE PK1 CLASSROOM.

Correction Required: A toilet area, training chair area, diapering area and sink area shall be equipped with a clean, lidded waste receptacle.

Noncompliance Area: ON THE AFTERNOON OF 2/1/17, STAFF MEMBER #1 SAT ON THE FLOOR, PLACED CHILD #1 ON THE FLOOR BETWEEN STAFF MEMBER'S LEGS AND EMBRACED THE CHILD FROM BEHIND. BECAUSE OF STAFF'S EMBRACE, THE CHILD COULD NOT MOVE ARMS. IN REPSONSE TO THIS, THE CHLD BEGAN KICKING LEGS. TO STOP THE CHILD FROM KICKING THE STAFF MEMBER, STAFF #1 PLACED OWN LEG OVER CHILD'S LEGS. IN RESPONSE, THE CHILD WAS CRYING AND BEGAN CALLING OUT FOR THE FATHER. IN ADDITION, CHILD #1 TRIED TO BREAK FREE FROM STAFF #1'S EMBRACE BY REPEATEDLY LIFTING UP OWN CHEST. THE CHILD WAS UNABLE TO MOVE ABOUT FREELY, AND TRIED TO BREAK FREE OF THE RESTRAINT. THE CHILD WAS CALLING OUT FOR ASSITANCE DUE TO THE RESTRAINT.

Correction Required: A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs.
