A safe indoor and outdoor environment was not provided for the children. A runner that is placed at the doorway to enter the classroom from the outside was not flat on the surface and is a tripping hazard to students and staff. The assistant director removed the runner from the deck.

Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. A plastic trash bag was hung on the fence to collect trash on the playground. The bag was hanging below 5 feet and was accessible to the toddler children that use that space. The bag was removed by the assistant director.
